An Article 51 experiment to provide better support for patients leaving hospital
This system proposes to intervene, within 6 working hours after the request of the health establishment, at the home of patients leaving hospital and whose state of health, level of autonomy and living conditions make it necessary to provide assistance and care at home from the first hours of their return to their home.
It will benefit from a derogation from the pricing and organization rules applicable to social and medico-social establishments and services, as part of the "Article 51" organizational and financial innovation experimentation system.
Three flexible packages are provided for this purpose:
- a 5-day "securitization" package, for an amount of € 614 ;
- a "transition" package of 15 days for an amount of € 1,744 ;
- a "reinforced" package of 30 days for an amount of € 3,113.
